→ Something people say

"I've got the prescription, so crossed out: the rest is up to me changed to: the rest gets a team too."

Already on treatment? Get the support around it.

Already on weight loss medication, and wondering who's looking at the whole picture? Here, a Nurse Practitioner leads your care from home, anywhere in Ontario. Your NP looks at what you're on the same way they look at the rest of your history, then builds a plan with you and keeps checking in.

The real answer

A prescription is one part of care. The rest deserves a clinician too.

You bring what you're on and how it's going. Your Nurse Practitioner goes through it: your history, your labs and your plan. Then you get a plan in writing that covers the routines, the side effects and what happens later, with check-ins that keep going. Nothing is assumed, either way, before your NP has looked.

  • What you're on and how it's goingyou
  • Your history, labs and plan, gone throughyour NP
  • Blood work near youlocal lab
  • Check-ins that keep goingyour care team

Nothing is assumed. Your NP looks first, then explains.

Joining with a prescription doesn't mean skipping the steps. Your Nurse Practitioner goes through your history by video, the same as for anyone else, with one more part: what you're on, how long you've been on it and how it's going for you.

Then your NP goes through it: your history, your labs and your plan. They tell you what they recommend and why, in plain words. That might mean keeping things as they are, changing something, or trying a different approach. Whatever it is, you hear the reason, and nothing changes without a conversation with you first.

Until then, don't change or stop anything on your own. Keep following the advice of the clinician who prescribed it.

  • Everything you take now, prescription or not, and roughly when each one started.
  • How it's been going: what's changed, what hasn't, and any side effects.
  • Recent lab results, if you have them. If not, your NP orders what's needed.
  • Your own goals, in your own words. Not every goal is a number.

The first step online is only how to reach you. What you're taking comes up later, with your Nurse Practitioner, not on a form.

Treatment works as part of a plan, not instead of one.

Canada's clinical practice guideline for adults living with obesity says care plans should address the root causes of weight gain and support changes to things like nutrition and activity, with treatments such as medication alongside them. It describes obesity as a chronic disease that "requires individualized treatment and long-term support like any other complex chronic disease."

So your plan isn't only about what you take. It's built from your actual week: sleep, work, food, movement and stress.

  • Routines that fit your real lifeNP + you
  • Support for side effectsyour care team
  • A plan for if treatment changes or stopsyour NP

Not seeing the progress you hoped for? That's worth a look.

If you're asking yourself "why am I not losing weight?", read why weight loss plateaus, then bring the question to your Nurse Practitioner.

How you feel is part of every check-in.

Check-ins cover how you feel, side effects, how the routines are working and whether the plan needs to change. Your labs and check-ins are how care stays safe, and what's checked is on Safety and monitoring.

And because treatment can change or stop for lots of reasons, your plan covers that from the start. More on the plan for later.

Good to know

  • Your NP is someone you can look up

  • Every visit happens while you're in Ontario

Video visits from home, and blood work at a lab near you. If something needs another specialist, your Nurse Practitioner arranges a referral.

  • If it isn't a fit, you'll know why

From your Nurse Practitioner, in plain words, with what to do instead. What happens if it's not a fit.

Questions, answered plainly.

I'm already on weight loss medication. Can I still join?

Yes, you can get started. Tell your Nurse Practitioner what you're taking and how it's going, and they'll go through it with you like the rest of your history.

Will you keep my current treatment going?

Your NP goes through it first: your history, your labs and your plan. Then they explain what they recommend and why, and nothing changes without a conversation with you.

Should I stop or change anything before my visit?

No. Don't change anything on your own; keep following the advice of the clinician who prescribed it until you've spoken with your Nurse Practitioner.

What if my weight loss medication doesn't seem to be working?

Bring it to your Nurse Practitioner. Progress can stall for many reasons, and why weight loss plateaus explains the common ones.

Is this only for people who aren't seeing results?

No. It's also for people whose treatment is going fine and who want a clinician, a plan and check-ins around it.

Do I need a referral?

No. You can get started without one.
