Your privacy matters most when the subject is your health. This page explains, in plain words, what we collect, why, where it goes and what you can ask us for.
Who we are
Ontario Virtual Weight Loss is an online medical weight loss program for adults in Ontario, led by a Nurse Practitioner.
There are two kinds of information here, and they're handled differently:
- Contact details you send through this website, so our team can reach you.
- Personal health information you share with your Nurse Practitioner during your care.
What the website form collects
The form on this site asks only how to reach you:
- your name;
- a phone number or an email address;
- the best time to reach you, if you choose one;
- a confirmation that you're 18 or older and live in Ontario;
- whether you'd like occasional news from Ontario Virtual Weight Loss (a separate box, unticked unless you tick it).
The form never asks for your weight, height, BMI, health history, conditions, medications, pregnancy status, date of birth or health card number. Please don't type health details into it. Those are for your conversation with a clinician.
We use these details to contact you about getting started. If you ticked the news box, we may also send occasional news, and you can unsubscribe at any time.
Where your contact details go
When you press send, your browser sends your details directly to the customer relationship system our team uses to follow up with new enquiries. They aren't sent by email. Your details are stored and processed by KremCRM on Microsoft Azure servers in Israel, outside Canada.
We use your contact details only to follow up on your enquiry.
Your health information, protected under PHIPA
Health information you share during your care, such as your history, your lab results and your plan, is personal health information under Ontario's Personal Health Information Protection Act, 2004 (PHIPA).
Your health information is used to provide your care. It's shared only as PHIPA allows, for example with a lab that runs your blood work or with another provider when you're referred.
Your rights
Under PHIPA, you have the right to ask to see the personal health information we hold about you, and to ask us to correct it if it's wrong. You can also ask what contact details we hold about you, and to correct them. And you can ask us to delete the contact details you sent through this website.

If you're not satisfied with how we've handled your information or a request, you can complain to the Information and Privacy Commissioner of Ontario.
Keeping your information safe
We keep your information securely, with access limited to the people who need it to contact you or care for you.
How this website is measured
We use Google Analytics to understand how the site is used: which pages are viewed, which buttons are pressed and whether a form was sent. It never receives your name, your contact details or any health information.
The BMI calculator works entirely in your browser. The numbers you enter and your result are never sent anywhere or stored.
Changes to this page
If we change how we handle information, we'll update this page.
